but it does require registration. I am currently working through activation problems.

BMJ archives
If you want to see an older article in the BMJ archives you need to use the "Print Issues" tab on their site.

They have plans to extend the archive back to the first issue, 1840, but currently it is available only back to 1994, and some of the older content is only in html.

JBJS American version
JBJS has just announced a service for your patients. They can get an article from JBJS American for free by going to www.JBJS.org and choosing the "patient access" option once they have located the particular PDF they desire. The article will be emailed to them.
